AGREEMENT & RELEASE OF LIABILITY: By accepting below I confirm that I have voluntarily registered to attend 5K Virtual Race for Lupus Awareness in May hosted by The Voice of Lupus Foundation. I am aware that my participation in this event INVOLVES NUMEROUS RISKS OF INJURY, including, but not limited to, falls, condition of the course, breathing difficulties, the effects of weather, loss of control, collisions with other participants, traffic and natural and man-made objects and I freely assume those risks. I confirm that I am physically fit and have trained appropriately for completion of this event. By signing this release, I consent to my voice, name, and/or likeness being used, without compensation, in any and all media in perpetuity. I voluntarily and forever waive, release and give up any and all claims, demands, liability, damages, costs and expenses of any kind whatsoever, including personal injuries to me or wrongful death against Event Organiser, all Event sponsors, and all Host Cities, Local Organizing Committees, Venues and Property Owners upon which the Event takes place, Law Enforcement Agencies and other Public Entities providing support for the Event, and each of their respective insurance companies, parent, subsidiary and affiliated companies, successors in interest, commercial and corporate sponsors, officers, directors, partners, shareholders, members, agents, employees, contractors, subcontractors, volunteers and TicketBae.com, a subsidiary of EMEL ('Ticket Bae') with respect to any liability, claims, demands, causes of action, damages, loss or expense (including court costs and reasonable attorneys’ fees) of any kind or nature, whether known or unknown (“Claim(s)”) from my presence at, or participation in, the Event, which may arise out of, result from, or relate to any alleged or actual NEGLIGENCE OF Event Organiser or breach of any express or implied warranty of the Released Parties. Parents/Guardian Waiver for Minors (under age 18): The undersigned parent and natural guardian does hereby represent that he/she is, in fact, authorized to act on behalf of and is acting in such capacity and agrees to save and hold harmless and indemnify each and all of the parties referred to above from all liability, loss, claims and damages. I agree to the use of my name and photograph in broadcasts, newspapers, brochures and other media without compensation. I acknowledge that providing my email address on the entry form entitles the event organiser and Ticketbae.com to email me regarding related topics.
